Dr. Sheppe often works with patients and families when the clinical picture has become layered, confusing, or fragmented across several diagnoses and treatments. He cares about identifying and addressing the root causes of difficulties. His approach considers symptoms alongside personality functioning, development, identity, relationships, family context, strengths, and the care environment around the patient.
Depending on the question, the work may include complex clinical evaluation, personality assessment, psychotherapy, psychoanalysis, transference-focused psychotherapy, medication management, family consultation, second opinions, and collaboration with an existing therapist, school, program, or treatment team.
The aim is a formulation that explains how the pieces fit together. That formulation can help clarify what has maintained the difficulty, what prior care may have missed, and what kind of treatment structure is most likely to make sense at this developmental moment.

Complex Clinical Evaluation
This work is useful when a presentation has accumulated multiple diagnoses, conflicting opinions, medication questions, or treatments that have helped only partly. Rather than reviewing symptoms in isolation, Dr. Sheppe aims at the root causes. He considers development, personality functioning, family dynamics, relationships, strengths, and the sequence of prior care.
The result is a more coherent formulation of what may be happening and why. That formulation can support diagnostic clarification, a change in treatment structure, or better coordination among the professionals already involved.
Personality Assessment & Development
Personality assessment is not simply a personality test or another label. It examines identity, self-esteem, emotion regulation, relationship patterns, coping patterns, strengths, and the overall level of personality functioning that may be shaping a person’s symptoms and treatment response.
This perspective can help distinguish what is situational, developmental, or more enduring. It can also clarify whether treatment should focus primarily on symptom relief, personality development, family participation, a more structured psychotherapy, or a different level of clinical support.
Transference-Focused Psychotherapy
Transference-Focused Psychotherapy is a structured psychodynamic treatment developed for severe personality difficulties in both adults and adolescents. The treatment pays close attention to patterns that emerge in the therapeutic relationship because those patterns often reflect the same conflicts affecting identity, emotions, and relationships outside therapy.
Dr. Sheppe is certified in TFP and completed several years of weekly individual supervision with its creator Otto Kernberg. His work may also involve parents, families, or treatment teams when the larger system is essential to stability and progress.
Adolescents & Emerging Adults
Adolescence and emerging adulthood can make diagnosis especially complex because development, identity, school or college, family expectations, relationships, and independence and autonomy are changing at the same time. Behaviors that look similar on the surface may reflect very different clinical needs.
Dr. Sheppe evaluates symptoms within that developmental context. Depending on the situation, care may include individual psychotherapy, family work, medication management, coordination with school or college supports, or collaboration with a broader treatment team.
Family Consultation
Clinical concerns often unfold within a larger family system. Parents, partners, and other relatives may be trying to help while also managing uncertainty, conflict, different expectations, or unclear roles in treatment. Family consultation creates space to understand how those patterns affect the patient and the care plan.
Dr. Sheppe may work with families alongside an adolescent, emerging adult, or adult patient, or consult with relatives when another clinician remains the primary therapist. The aim is to improve communication, clarify boundaries and responsibilities, support treatment participation, and help the family respond in ways that strengthen rather than unintentionally complicate the clinical work.
Failure to Launch
Young adults who appear stuck may be contending with mood or anxiety symptoms, ADHD, trauma, personality difficulties, avoidance, family patterns, or a mismatch between expectations and developmental capacity. The root causes of the situation need to be understood.
Dr. Sheppe evaluates what may be maintaining dependence, withdrawal, conflict, or repeated breakdowns around school, college, work, relationships, and daily responsibility. The goal is a realistic treatment direction that supports greater autonomy while accounting for the young adult’s current clinical and developmental capacity.
Integrated Psychiatry & Psychotherapy
Some patients need more than a brief medication visit, while others already have a therapist and need focused psychiatric consultation. Dr. Sheppe can hold medication questions, psychotherapy, psychoanalysis, family context, and team coordination inside one formulation rather than treating them as unrelated services.
This may mean providing both medication management and psychotherapy, consulting while another clinician remains the primary therapist, or helping a multidisciplinary team organize competing perspectives into a clearer plan.
